Mpox multi-country outbreaks

Mpox multi-country outbreaks is one continuing Orthopoxvirus event: the 2022 clade IIb spread through sexual networks in countries that had not previously reported sustained human-to-human mpox, the International Health Regulations (2005) Public Health Emergency of International Concern determined on 23 July 2022, its termination on 11 May 2023, then the 2024 upsurge of clade I (including clade Ib) in the Democratic Republic of the Congo and neighbouring States…

What this driver is

Mpox multi-country outbreaks is one continuing Orthopoxvirus event: the 2022 clade IIb spread through sexual networks in countries that had not previously reported sustained human-to-human mpox, the International Health Regulations (2005) Public Health Emergency of International Concern determined on 23 July 2022, its termination on 11 May 2023, then the 2024 upsurge of clade I (including clade Ib) in the Democratic Republic of the Congo and neighbouring States, a second PHEIC determined on 14 August 2024 and terminated on 5 September 2025. It is a mechanism with dated IHR instruments, not a temperament, not a 2022-only “gay disease” headline, and not a unique Factrail case count.

This driver is not COVID-19 pandemic, not pandemic preparedness, not infectious-disease transmission, and not vaccine accessibility. Those live rows are a different virus or metric companions. A later standing recommendation does not mint a twin.

It applies from the May 2022 multi-country detections through both PHEIC windows and the post-2025 programmatic phase. It is not smallpox eradication, not COVID-19, not Ebola as a substitute, and not ukraine-war. Endemic Central and West African zoonotic circulation before 2022 is neighbouring history. It is not this driver’s start date.

The legal core is the IHR (2005). The first Emergency Committee, 23 June 2022, advised that the event did not constitute a PHEIC. The Director-General, after a divided second meeting on 21 July 2022, determined a PHEIC on 23 July 2022. WHO renamed the disease “mpox” in November 2022. Africa CDC declared a public health emergency of continental security on 13 August 2024, the day before the second WHO PHEIC. Ending a PHEIC is not eradication.

Mechanism

The outbreaks move welfare through named channels.

First, human-to-human spread. Clade IIb in 2022 moved mainly through sexual contact among men who have sex with men in newly affected countries. Clade I in 2024 moved in eastern DRC households, children, and sexual networks, then into Burundi, Kenya, Rwanda, and Uganda. A PHEIC is a coordination instrument. It is not a vaccine.

Second, delayed alarm and neglected endemic burden. The June 2022 Committee did not recommend a PHEIC while cases were already in dozens of countries. Dimie Ogoina, chairing the 2024 committee, said mpox originating in Africa was neglected there and later caused a global outbreak. That is a dated criticism of the architecture, not a Person page.

Third, countermeasures and inequity. MVA-BN / JYNNEOS and LC16m8 doses, tecovirimat, and diagnostics concentrated in high-income 2022 programmes. DRC and neighbours in 2024 still faced testing gaps: WHO noted clinically compatible cases untested. Gavi announced First Response fund availability after the 2024 PHEIC. Dose announcements are not coverage. This page does not invent a unique vaccine-delivery integer.

Fourth, stigma and naming. “Monkeypox” and MSM-only framing delayed community protection and hid paediatric clade I deaths. WHO’s November 2022 rename is a completed vocabulary act. It does not rebate 2022.

Fifth, exit and split alarms. On 5 September 2025 WHO terminated the second PHEIC on sustained declines in DRC, Burundi, Sierra Leone, and Uganda. Africa CDC the previous day extended its continental emergency. Two official instruments, same week, different scope. This page stores both. It does not pick a unique Factrail incidence.

Timing is days for a sexual-network cluster and months for a household paediatric wave. Ending a PHEIC does not erase 2022 or 2024.

Evidence base

The best evidence is IHR committee reports and Director-General determinations.

Second IHR Emergency Committee, 21 July 2022; PHEIC determined 23 July 2022 (https://www.who.int/news/item/23-07-2022-second-meeting-of-the-international-health-regulations-(2005)-(ihr)-emergency-committee-regarding-the-multi-country-outbreak-of-monkeypox (opens in a new tab)). The Director-General’s press statement the same day records that the first meeting had resolved the outbreak was not a PHEIC, then more than 16 thousand reported cases from 75 countries at the second determination (https://www.who.int/news-room/speeches/item/who-director-general-s-statement-on-the-press-conference-following-IHR-emergency-committee-regarding-the-multi--country-outbreak-of-monkeypox--23-july-2022 (opens in a new tab)). Those are WHO’s contemporaneous figures.

Fifth meeting on the 2022 multi-country outbreak, PHEIC terminated 11 May 2023 (https://www.who.int/news/item/11-05-2023-fifth-meeting-of-the-international-health-regulations-(2005)-(ihr)-emergency-committee-on-the-multi-country-outbreak-of-monkeypox-(mpox) (opens in a new tab)). Second PHEIC determined 14 August 2024 for the DRC-centred upsurge (https://www.who.int/news/item/14-08-2024-who-director-general-declares-mpox-outbreak-a-public-health-emergency-of-international-concern (opens in a new tab)). Fifth meeting on the 2024 upsurge; PHEIC terminated 5 September 2025 (https://www.who.int/news/item/30-10-2025-fifth-meeting-of-the-international-health-regulations-(2005)-emergency-committee-regarding-the-upsurge-of-mpox-2024 (opens in a new tab)).

The meter on this driver is IHR PHEIC in force: 1.0 on 23 July 2022, 0.0 on 11 May 2023, 1.0 on 14 August 2024, 0.0 on 5 September 2025. That is a documented transformation of four official determinations, not an invented case curve. WHO and Africa CDC death and case tallies in sitreps remain named edge instruments. This page does not invent Contribution.size or a unique DRC paediatric death total.

Evidence is strongest for a signed PHEIC determination and a published committee report. It is weaker for any unique first-week case coefficient. It is weakest where a later standing recommendation is asked to rebate 2022 vaccine inequity.

Effect on welfare indicators

This driver, when it actually runs, is a channel into child survival where clade I is severe, and into service coverage where testing and vaccines fail to arrive.

  • Under-five mortality rate: direction higher deaths when clade I household transmission reaches young children in DRC and neighbours. Horizon: weeks to months of a wave. Confidence: medium for the paediatric channel described in WHO clade I notes; this page does not invent a unique under-five mpox integer. The series is lower-is-better, so strengthening the outbreak is welfare-negative.
  • Universal health coverage (UHC) service coverage index: direction lower effective coverage when diagnostics, isolation, and vaccines are missing in the same districts that report clinically compatible cases. Horizon: months. Confidence: medium for access failure as a coverage channel; low for a global UHC coefficient. Strengthening the outbreak without countermeasures weakens a higher-is-better series.

A measles or PISA path does not follow from mpox without a sourced channel. Missing coefficients stay missing.

Interactions and contested points

Mpox interacts with infectious-disease transmission, vaccine accessibility, health-system capacity, and international health cooperation. Reinforcement is obvious when a new clade, a testing gap, and a delayed PHEIC hit the same province. Offsetting is the claim that a 2022 high-income JYNNEOS campaign or a 2025 PHEIC termination cancels DRC paediatric burden. Those are different instruments. They do not erase 14 August 2024.

The strongest competing view is that 2022 clade IIb and 2024 clade I are two diseases and should be two drivers, or that endemic African mpox is “always there” so a PHEIC was theatre. The competing defence of this node is the IHR record: two PHEIC determinations, one rename, one neglected-endemic criticism from the 2024 chair, and Africa CDC’s continental emergency overlapping WHO’s second PHEIC. Two clades, one continuing event.

A second competing view is that ending the 2025 PHEIC while Africa CDC kept a continental emergency proves WHO left too early. This page stores both instruments. It does not invent a unique 2025 case curve to break the tie.

Established: first Committee 23 June 2022 did not advise a PHEIC; PHEIC 23 July 2022; rename November 2022; PHEIC end 11 May 2023; Africa CDC continental emergency 13 August 2024; WHO PHEIC 14 August 2024; WHO PHEIC end 5 September 2025. Not established: a unique Factrail case or death total, or that a standing recommendation to August 2026 is eradication. A later clade with a new PHEIC date would be a new fact on this same driver.
