COVID-19 pandemic
The COVID-19 pandemic is one continuing civilizational health shock: SARS-CoV-2 from the late-2019 cluster reports, the International Health Regulations (2005) Public Health Emergency of International Concern determined on 30 January 2020, the WHO Director-General’s 11 March 2020 characterization of COVID-19 as a pandemic, years of transmission, hospital overload, missed routine immunization and vaccine nationalism… Its strongest welfare pathway: it weakens Life expectancy at birth, a worse welfare reading.

What this driver is
The COVID-19 pandemic is one continuing civilizational health shock: SARS-CoV-2 from the late-2019 cluster reports, the International Health Regulations (2005) Public Health Emergency of International Concern determined on 30 January 2020, the WHO Director-General’s 11 March 2020 characterization of COVID-19 as a pandemic, years of transmission, hospital overload, missed routine immunization and vaccine nationalism, then the 5 May 2023 determination that COVID-19 no longer constituted a PHEIC while remaining an established health issue.
This driver is not pandemic preparedness, not infectious-disease transmission, not vaccine accessibility, not mRNA platform development, and not health-system capacity. Those live rows are metric companions. This node is the 2019– event itself. A later IHR amendment or Pandemic Agreement does not mint a twin.
It applies worldwide from December 2019 cluster notification through the PHEIC window (30 January 2020–5 May 2023) and the post-PHEIC phase. It is not 1918 influenza, not HIV/AIDS as a substitute, not mpox, and not ukraine-war. Neighbouring mechanisms can move with this shock. They do not replace it.
The legal core is the IHR (2005), PHEIC Temporary Recommendations, IHR amendments adopted 1 June 2024 (WHA77.17), and the WHO Pandemic Agreement adopted 20 May 2025 (WHA78.1), which is not yet in force pending the Pathogen Access and Benefit-Sharing annex and ratifications. A press-conference adjective does not rewrite those texts.
Mechanism
The pandemic moves welfare through named channels.
First, infection and severe disease. Transmission, hospital occupancy, and disruption of other care raise mortality. Reported COVID-19 deaths to WHO are one official lens. They undercount. WHO’s 5 May 2022 excess-mortality instrument estimated about 14.9 million excess deaths associated with the pandemic in 2020–2021, against a much smaller reported COVID-death tally in the same window. Those two official series disagree.
Second, emergency law. Stay-at-home orders, border closures, school shutdowns, and workplace rules changed who could work, learn, and reach a clinic. The European Council’s 17 March 2020 backing of a 30-day restriction on non-essential travel into the Union is a dated regional instrument. A restriction is not a vaccine.
Third, medical countermeasures and unequal delivery. The U.S. FDA issued the first emergency use authorization for the Pfizer–BioNTech COVID-19 vaccine on 11 December 2020. WHO issued an Emergency Use Listing on 31 December 2020. COVAX’s first international shipment landed in Accra on 24 February 2021 (Serum Institute of India / Oxford–AstraZeneca doses). High-income advance-purchase contracts still outran AMC delivery.
Fourth, information control and origin dispute. Early local suppression of clinicians in Wuhan, delayed international notification, and later lab-leak versus zoonosis arguments remain contested. The WHO-convened China joint study (March 2021) called a laboratory incident “extremely unlikely.” Later assessments disagreed. This page stores the dispute as a dispute. It does not mint an origin verdict or a Person page for the WHO Director-General.
Fifth, exit shocks. China’s Joint Prevention and Control Mechanism “ten new measures” circular of 7 December 2022 (联防联控机制综发〔2022〕113号) ended mass testing and centralized isolation for mild disease without a staged vaccine-first elderly campaign. A sudden exit is a dated act, not a tourism brochure.
Timing is days for a hospital wave and years for missed measles doses and post-COVID condition. Ending a PHEIC does not erase 2020–2022.
Evidence base
The best evidence is IHR and WHA instruments plus independently reported official series.
WHO determined a PHEIC on 30 January 2020 (IHR Emergency Committee, novel coronavirus).
The meter is WHO’s reported New_deaths field in the public global CSV (https://srhdpeuwpubsa.blob.core.windows.net/whdh/COVID/WHO-COVID-19-global-data.csv (opens in a new tab)), summed across countries by year and divided by the 2021 peak of 3,549,358 reported new deaths. 2020 is 1,897,533 (0.5346), 2021 is 1.0, 2022 is 1,248,805 (0.3518), 2023 is 318,570 (0.0898), 2024 is 67,861 (0.0191), 2025 is 26,889 (0.0076). WHO has said reported deaths undercount. The 14.9 million 2020–2021 excess-death estimate (WHO news, 5 May 2022) is a named competing instrument. It is not copied onto this meter.
FDA’s 11 December 2020 EUA (https://www.fda.gov/emergency-preparedness-and-response/coronavirus-disease-2019-covid-19/covid-19-vaccines (opens in a new tab)) and Gavi’s 24 February 2021 Accra shipment are the countermeasure edges. China’s 7 December 2022 circular is on gov.cn (https://www.gov.cn/xinwen/2022-12/07/content_5730443.htm (opens in a new tab)). Donald Trump’s first-term WHO withdrawal is United States notifies WHO of first-term withdrawal; the 20 January 2025 restart is President Trump initiated a new United States withdrawal from WHO. WHA77.17 (1 June 2024) and WHA78.1 (20 May 2025) are the after-action layer (https://www.who.int/news/item/20-05-2025-world-health-assembly-adopts-historic-pandemic-agreement-to-make-the-world-more-equitable-and-safer-from-future-pandemics (opens in a new tab)). Adopted ≠ in force.
Evidence is strongest for a signed IHR determination, an FDA EUA letter, a Gavi shipment notice, and a State Council circular. It is weaker for any unique first-wave death coefficient. It is weakest where a later Assembly vote is asked to rebate 2020.
Effect on welfare indicators
This driver, when it actually ran, was a first-order channel into survival and missed prevention.
- Under-five mortality rate: direction higher deaths when infection, interrupted obstetric and paediatric care, and missed immunization coincide. Horizon: weeks to a few years. Confidence: medium in high-transmission, low-capacity settings. The series is lower-is-better, so strengthening the pandemic is welfare-negative. No unique under-five COVID integer is invented here.
- First-dose measles vaccination coverage: direction lower coverage when campaigns and clinics stopped. Horizon: months to years. Confidence: high that 2020–21 produced documented campaign pauses. Strengthening the pandemic weakens a higher-is-better series. Missed-dose stocks stay on the edge instrument.
- Life expectancy at birth: direction lower period life expectancy in 2020–21 where vital registration captured the shock. Horizon: same years for period measures. Confidence: high that a large mortality shock moves period LE; low for a unique global coefficient.
Missing coefficients stay missing.
Interactions and contested points
The pandemic interacts with vaccine hesitancy, disinformation prevalence, trust in public institutions, and international health cooperation. Reinforcement is obvious when a wave, a closed clinic, and a rumour hit the same district. Offsetting is the claim that a fast mRNA authorization, a COVAX landing, or a 2025 Assembly vote cancels 2020. Those are different instruments on different dates. They do not erase the PHEIC years.
The strongest competing view is that reported COVID deaths are the wrong meter because excess mortality was several times larger, or that “the pandemic is over” as of 5 May 2023 so this node should close. WHO publishes both series and said ending the PHEIC does not mean COVID-19 is over as a health threat. A disclosed, scaled official meter plus a named contested excess instrument is more honest than inventing a Factrail body-count.
A second competing view is that origin is a laboratory incident and that IHR reform is a sovereignty grab. What WHA77.17 and WHA78.1 established is a package of amendments and an adopted-not-in-force agreement. What they did not establish is a completed origin finding or a WHO power to impose lockdowns. Origin remains unresolved here.
Established: PHEIC 30 January 2020; pandemic characterization 11 March 2020; FDA EUA 11 December 2020; COVAX Accra landing 24 February 2021; China ten measures 7 December 2022; PHEIC end 5 May 2023; IHR amendments 1 June 2024; Pandemic Agreement adopted 20 May 2025, not in force. A later PABS annex with a date would be a new fact.
