On 6 February 1594 John Danter entered Titus Andronicus. A 1594 quarto survives. Henslowe's Diary also records early performances by other companies. The Fact here is the 1594 printed play associated with Shakespeare.
Context
Titus is an early, spectacularly violent tragedy. Later stylometric work assigns part of the play to George Peele. That is an authorship hypothesis sitting beside the Fact, not a reason to split the quarto into two publications.
Why it matters
Early print plus early staging is the beginning of Shakespeare's public dramatic corpus, before the Chamberlain's Men patent.

What the sources establish
The unique 1594 quarto described by the British Library, plus the Stationers' entry, establish the book. Henslowe's Diary, an Admiral's/Strange's account book, is independent theatre evidence of a Titus on stage. Neither source measures crime rates. Wikipedia is not used as a causal source.
Established: 1594 entry and quarto. Not established: sole authorship of every scene; that the play caused public violence. Confidence high on the book, medium on exclusive authorship.
The event remains dated 1594-02-06. An early printed tragedy put a violent Roman play into both yard and bookstall. Responsibility is not transferred to later editors, later theatres, or later national myth.
